What to Bring to Your Office Visit

The office is open Monday through Friday from 9:00 am until 5:00 pm.
There are evening office hours on Monday, Tuesday, and Wednesday from 6:00 pm until 8:00 pm.
The office is open to see urgent cases on Saturday mornings from 9:00 am until 12:00 pm.
Healthcare providers are accessible off-hours. Please call the main office number.

What to Bring to Your Office Visit back to top
We would like to make your office visit as pleasant an experience as possible.
- Please bring your insurance card
- Bring any forms you need to be filled out (i.e. school physical forms, sports phycial forms) to avoid paying a surcharge.
- If this is your first visit to the office, please bring any medical records you may have. If you do not have the records yourself, please bring a listing of doctors you have visited along with their contact information.
- Please bring any prescription medications in original bottles if possible.
- If you have access to a printer, please complete the Patient Registration Form and Medical History Form (274 KB) prior to your visit. You will need to have Adobe Acrobat software to fill out the Medical History Form. If your computer does not have Adobe Acrobat installed you may download it for free by clicking on the "Get Acrobat Reader" icon below.
